Case management and support will be provided by the Triage physiotherapist for each employee from referral through to discharge regardless of the stepped care pathway taken. This ensures a constant point of contact for employee, line manager and Occupational Health as appropriate.
Initial assessment report and plan
Authorisation of a treatment plan (where to be provided by a subcontracted clinic)
Authorisation of additional treatment sessions (if required)
Production and maintenance of a return to work plan (shared with employee and line manager)
Follow up calls with the employee to review progress and appropriateness of treatment (colleagues will be routed to another stepped care pathway if deemed appropriate)
Discharge report and plan
